The deadline to upload proof of immunizations is based on the semester for which a student was admitted*:

Your registration may be canceled if you are not compliant with the University's immunization requirements. Forms will be processed within 48 hours of receipt, and immunization status can be verified via MyUHealthChart.com as outlined below.
Most students will be able to obtain the required immunization information from their prior medical providers or from their prior high school, college, or university. Students who believe that they were previously immunized but are unable to provide proof of immunization may either obtain blood tests confirming immunity or obtain the necessary immunizations. Immunizations and blood tests documenting immunity are available at the Student Health Service.
For students less than 18 years old, the meningitis/hepatitis vaccine waiver must be signed by a parent or legal guardian.
Once completed and signed by a healthcare provider, upload your immunization form either from your computer or mobile device (phone, tablet, etc).
Nursing, Medical and Physical Therapy students must complete the annual tuberculosis screening questionnaire once a year. Students who answer 'yes' to any of the questions on the screening form should schedule an appointment with a Student Health provider to discuss the need for further testing. Immunization information is provided to the State of Florida FLORIDA SHOTS immunization registry. Students can opt out of the immunization registry by completing the Florida Department of Health's opt-out form and submitting it to the health department as outlined on the form. This is an opt-out of sharing immunization information with the State of Florida registry and not an opt-out of the immunization requirement.
