Immunization Submission Deadlines

The deadline to upload proof of immunizations is based on the semester for which a student was admitted*:

Fall: July 25
Spring: December 15
Summer: April 15

*Medical student deadlines are as stated on the medical student immunization form provided below.

Your registration may be canceled if you are not compliant with the University's immunization requirements. Forms will be processed within 48 hours of receipt, and immunization status can be verified via MyUHealthChart.com as outlined below.

Completing and Uploading the Immunization Form

Most students will be able to obtain the required immunization information from their prior medical providers or from their prior high school, college, or university. Students who believe that they were previously immunized but are unable to provide proof of immunization may either obtain blood tests confirming immunity or obtain the necessary immunizations. Immunizations and blood tests documenting immunity are available at the Student Health Service.

For students less than 18 years old, the meningitis/hepatitis vaccine waiver must be signed by a parent or legal guardian.

Once completed and signed by a healthcare provider, upload your immunization form either from your computer or mobile device (phone, tablet, etc).
How to Upload Immunization Form
1. Go to MyUHealthChart.com.
2. Log in with your credentials.
3. Click on Menu on the top left, and navigate down to the Student Health Services section to click on Document Submission.
4. Click on Add a Document under the Immunization Record heading.
5. Please allow up to 2 business days for processing.
How to Verify Compliance
1. Go to MyUHealthChart.com
2. Login with your credentials.
3. Click on Menu on the top left, and navigate down to the Student Health Services section to click on Student Immunization Requirements.
4. Under the Compliant with Current Requirement section, a "Y" signifies that you are compliant with your immunization requirements. An "N" signifies that you are not compliant with your immunization requirements. Please review the section below to determine what you are missing and upload the appropriate documentation.
Health Science Students Annual Tuberculosis Screening
Nursing, Medical and Physical Therapy students must complete the annual tuberculosis screening questionnaire once a year.

Students who answer 'yes' to any of the questions on the screening form should schedule an appointment with a Student Health provider to discuss the need for further testing.

Students who do not have any symptoms are not required to undergo yearly tuberculosis testing.

How to complete your annual tuberculosis screening questionnaire:
1. Go to MyUHealthChart.com.
2. Login with your credentials.
3. Click on Menu on the top left, and navigate down to the Student Health Services section to click on Annual TB Questionnaire.
4. Answer the question and click Continue.
5. Review your selections and then click Submit.

About Florida Shots

Immunization information is provided to the State of Florida FLORIDA SHOTS immunization registry. Students can opt out of the immunization registry by completing the Florida Department of Health's opt-out form and submitting it to the health department as outlined on the form.

This is an opt-out of sharing immunization information with the State of Florida registry and not an opt-out of the immunization requirement.
