Class Excuse Policy

Students may experience medical or psychological conditions, illnesses, or injuries that require them to be away from the classroom/workplace or that impact their ability to perform optimally in class or on exams.

Student Health Service can confirm dates that a student is excused from attending in-person classes/work shifts based on the nature of the student's situation. However, private health information will never be disclosed nor will information be shared until the student provides Student Health Service with authorization to disclose these dates. 

Students are encouraged to notify their instructors/supervisors as soon as possible to develop a plan to make up missed assignments/tasks.

Routine Medical or Illness-Related Visits

Students can request a note from their healthcare provider to verify routine medical visits or for care related to a specific illness/injury. To request a note from your provider, log in to MyUhealthChart and select "Communicate with your doctor" from the menu.


COVID-19 Isolation/Quarantine

Students who need to confirm with their faculty/supervisor of an absence from in-person class/work due to isolation/quarantine or faculty/supervisors looking to confirm a student's absence can submit the Absence Verification form.
