Patient's Rights & Responsibilities

You have the right to:

  • Be treated with dignity, respect, courtesy, and privacy.
  • Receive information and explanation concerning diagnosis, treatment and prognosis.
  • Refuse treatment and be informed of the consequences of the decision.
  • Expect that medical records will be kept confidential and released only with your written consent or in response to court-ordered subpoenas.
  • Select and/or change your health care provider.
  • Receive appropriate referrals to other providers and services.
  • An explanation of any charges incurred for services rendered.
  • Know the name, position, and status of all individuals working at the Health Center.

You have the responsibility to:

  • Provide complete and accurate information pertaining to your medical history and current medical problem.
  • Understand your health problems and participate in the plan of care. Ask for clarification if instructions are not understood.
  • Understand your providers recommendations for continuing care.
  • Keep an appointment or call if you cannot meet your scheduled appointment.
  • Show courtesy and respect to Health Center staff and all other clients.
  • Communicate with your health care provider if your condition worsens or does not follow the expected course.
